Registered/Enrolled Nurse - General Practice

Leschenault Medical Centre is looking for an experienced Care Plan Nurse to join our growing team. The position is part-time (3 days per week) and would suit a friendly and dedicated Registered or Nurse or Enrolled Nurse.

Tasks and Responsibilities:

• Conducting patient triage effectively.

• Contributing to Chronic Disease Management Plans and Health Assessments.

• Delivering comprehensive patient education and support.

• Providing crucial assistance to General Practitioners in emergency and injury management.

• Maintaining a well-organised treatment room and efficiently managing patient flow.

• Managing and controlling medical stock levels.

  
  

Essential:

• Current and unrestricted AHPRA nursing registration.

• A strong patient-centric approach with exceptional interpersonal, communication, and customer service skills.

• Proven ability to work collaboratively within a multidisciplinary team, demonstrating excellent teamwork and multitasking capabilities.

• Proficiency in using various clinical and scheduling software, requiring strong computer literacy.

• A positive and proactive "can-do" attitude is essential to thrive within our friendly and supportive environment.

Highly Regarded:

• Previous experience working in an accredited General Practice setting.

• Familiarity with clinical software such as Best Practice, Medtech, or similar systems.

• Possession of a completed Immunisation Certificate or actively working towards completion.

• Experience in care planning and chronic disease management.

Leschenault Medical Centre is a busy General Practice, we currently have a team of 2 nurses, 11 GPs plus allied health. Our mission is to provide comprehensive and thoughtful medical care to individuals and families.
