We are seeking a dedicated, compassionate and community-focused Family Support Coordinator to support children and families to successfully engage in learning and school life. Working collaboratively with classroom teachers, school support staff and Early Childhood Coordinators, you will help create safe, welcoming and inclusive learning environments and support children and families as they transition into school.

Harlaxton State School is a small, welcoming school in northern Toowoomba with a strong history and valued partnerships across the local community. Your Opportunity
As the Family Support Coordinator, you will work with classroom teachers, school support staff, Early Childhood Coordinators (where available) to establish safe, welcoming learning environments.
Coordinators will assist the school team to support children and families successfully transition into school from early childhood services, facilitate access to learning or engagement support to help establish positive patterns of school attendance, behaviour and engagement in learning and link families to external government and non-government supports and services to reduce barriers to learning for students.
The role encompasses two locations:
Toowoomba North State School (base school) - 0.50FTEHarlaxton State School - 0.50FTE
Responsibilities
Build strong partnerships with families, schools, government agencies and community services.Support students and families with school transition, attendance, behaviour and engagement.Connect families with appropriate health, therapeutic, developmental and specialist services, including NDIS supports.Work with teachers and support staff to identify student needs and appropriate interventions.Support vulnerable children and families and strengthen family capacity and wellbeing.Develop local networks and coordinate multi-agency support.Use data to monitor the effectiveness of support strategies and improve student outcomes.Maintain accurate case and activity records in accordance with legislation and departmental requirements.
