The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department.

Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.

Social Work provides a range of services including:

• Counselling
• Discharge Planning
• Crisis interventions
• Bereavement counselling
• Family support
• Community liaison

The Role

A fantastic opportunity exists for an experienced Social worker to work as the NDIS Social work Navigator.

Alfred Health is home to Australia’s busiest trauma centre and also delivers numerous community-based programs. This role will work across the Trauma Social work service providing advanced Social work services to patients and their families/significant others across the Trauma stream. The role will also work in supporting participants who are receiving support and care from the NDIS. 

You will have demonstrated prior experience in managing a complex clinical caseload and will have sound knowledge and skills in hospital discharge planning and working with patients on the NDIS. An extensive understanding of the barriers for patients on the NDIS within the health service context will be essential for this role. This role will be required to provide secondary consultation and support to staff who are supporting patients on the NDIS. An understanding of Data capture and improving the quality of care for patients on the NDIS will be required. You will possess strong assessment, case planning, and report writing experience and have an understanding of discharge planning which will be essential to this role. You will work within a strong multidisciplinary team and will be part of an organisation where professionalism, innovation and the ability to find creative solutions to challenges is highly regarded. This role will work closely with the Disabilty Team to provide a coordinated care response to patients.

The role will involve providing comprehensive psychosocial assessments, crisis intervention, adjustment counselling, case planning, family work, community liaison, networking and referral to external stakeholders and discharge planning. The successful applicant will need to contribute to quality improvement and research activities and provide staff and student supervision.   

Experience and Qualifications Required

• Applicants must hold a Bachelor's Degree in Social Work
• Applicants must have an understanding of the psychosocial issues associated with this patient cohort
• Substantial experience in working within a hospital setting will be essential to this role  
• Experience in working within an Electronic medical record is vital
• Extensive knowledge of the NDIS and hospital discharge planning within a health service 
• Working knowledge and understanding of the role of the NDIS Health Liaison Officer will also be crucial for this role
