Social Worker Senior

Provide advanced clinical social work leadership while supporting complex care journeys and improving outcomes across acute and subacute services.

About the role

The Social Work Senior provides high-quality clinical leadership and evidence-informed social work services within the Social Work Service at Redcliffe Hospital. Working across acute and subacute healthcare settings, the role supports patients and families experiencing complex psychosocial issues, safeguarding concerns and care transition challenges, while contributing to positive patient outcomes through expert assessment, intervention and multidisciplinary collaboration.

The successful candidate will manage complex clinical presentations, provide advanced psychosocial assessment and therapeutic interventions, and contribute to service development, education and quality improvement initiatives. The role also supports workforce capability through supervision, mentoring and training activities, fostering excellence in clinical social work practice and continuous improvement in service delivery.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

• Deliver high-level evidence-informed social work services, including psychosocial assessment, therapeutic intervention, safeguarding and complex care coordination across acute and subacute healthcare settings.
• Apply advanced clinical judgement and independent decision-making to manage complex psychosocial, discharge planning and care transition matters.
• Provide expert advice and collaborate effectively with multidisciplinary teams, patients, families and external agencies to achieve safe, person-centred outcomes.
• Provide clinical supervision, education and mentoring to social workers, students and other healthcare professionals to support workforce development and clinical excellence.
• Lead service improvement, quality, research and portfolio activities that enhance social work practice, safeguarding outcomes and patient care.
