About the role
We are seeking a compassionate and self-motivated Community Mental Health Practitioner to join our Transitional Recovery Program across Rockhampton and Yeppoon. In this role, you will provide recovery-focused support within a residential setting, helping people build confidence, develop daily living skills, strengthen community connections, and work towards independent living.
About the service
The Transitional Recovery Program (TRP) operates across two residential houses in the Rockhampton region and Yeppoon, supporting adults transitioning from acute mental health settings back into the community and reducing the need for further hospital admissions.
The service provides 24/7 support in a safe, home-like environment through one-to-one support, shared supports, and group activities tailored to individual recovery goals.
