About the role

We are seeking a compassionate and self-motivated Community Mental Health Practitioner to join our Transitional Recovery Program across Rockhampton and Yeppoon. In this role, you will provide recovery-focused support within a residential setting, helping people build confidence, develop daily living skills, strengthen community connections, and work towards independent living.

About the service

The Transitional Recovery Program (TRP) operates across two residential houses in the Rockhampton region and Yeppoon, supporting adults transitioning from acute mental health settings back into the community and reducing the need for further hospital admissions.

The service provides 24/7 support in a safe, home-like environment through one-to-one support, shared supports, and group activities tailored to individual recovery goals.

Key Responsibilities

• Support clients to transition safely back into the community and work towards independent living.
• Deliver high-quality recovery-oriented support and positive social experiences.
• Work as part of a multidisciplinary team offering specialised psychosocial support.
• Plan and deliver group programs that aid clients to build their skills and focus on recovery.
• Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.
• Work across a 24/7 rotating roster, including sleepovers, weekends, and public holidays.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, community services, Disability, Youth Work, Psychology, Social Work, or other health-related field (or willingness to work toward this).

• Experience working with people with mental health challenges and complex needs, including families and carers.
• Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma- informed care and family inclusive practice.

• Current First Aid and CPR certificates.
• Ability to provide proof of vaccination against vaccine preventable diseases or willingness to obtain required vaccinations per Queensland Health partnership.
