Occupational Therapist, Home Based Care, Southern Sydney
This role is 100% home based, supporting people in aged care and people living with disability across Sydney's southern suburbs. You'll be joining a mobile allied health provider with an established, collaborative multidisciplinary team.
About The Role
100% adult caseload, made up of aged care (Support at Home / CHSP) and NDIS disability supportHome safety assessments, assistive technology prescription, home modifications and functional capacity reportingAppointments scheduled within roughly a 30 minute drive of your own home baseHome based role, with occasional access to our clinic offices for clinic based work if neededMonthly in person team meetings held at one of our clinic locations
Key Responsibilities
Conduct comprehensive assessments and develop individualised intervention plans for adult clientsPrescribe and review assistive technology, equipment and home modificationsPrepare clear, well documented reports and recommendationsLiaise with support coordinators, case managers, families and other allied health providersContribute to a collaborative multidisciplinary team alongside Physiotherapists, Exercise Physiologists, Speech Pathologists and fellow OTs
The team
You'll join a wider Occupational Therapy and Speech Pathology team of around 10 cliniciansThe broader clinical team includes around 12 Physiotherapists and Exercise Physiologists, supported by 3 administrative staff who manage scheduling and non-clinical logisticsThe full team meets fortnightly, alternating between in person and online sessions
Requirements
Degree in Occupational Therapy with current AHPRA registrationComfortable working autonomously in clients' homes, with sound clinical reasoning and reporting skillsFull time preferred, though 3 to 4 days a week can be considered for the right candidateOwn car and driver's licence
What's On Offer
$80,000 to $85,000 base salary, depending on experience$1.01 per km car allowance, plus tolls and parking reimbursed (no car is provided, but running costs are fully covered)
