Trinity Public Care Pty Ltd is seeking a Care Coordinator and Business Development Manager – NDIS to join our growing team in Sydney, NSW. This is a full-time position that combines care coordination responsibilities with business development initiatives to support our expansion within the National Disability Insurance Scheme (NDIS) sector.
In this pivotal role, you will bridge the gap between direct service delivery and organisational growth. You will be responsible for coordinating high-quality disability support services whilst simultaneously identifying and developing new business opportunities to strengthen our market position. This role is essential to achieving our strategic objectives in the aged and disability support industry.
Key responsibilities
Coordinate individualised support plans and services for NDIS participants, ensuring quality outcomes and compliance with NDIS requirements
Assess participant needs and develop tailored care coordination strategies that align with their goals and aspirations
Liaise with NDIS planners, participants, families and support workers to facilitate seamless service delivery
Manage participant documentation, records and reporting to maintain regulatory compliance and evidence of service provision
Identify and evaluate new business opportunities within the disability support and aged care sectors
Develop and implement strategies to expand our service offerings and increase market penetration
Build and maintain relationships with key stakeholders, including NDIS planners, community organisations and potential partners
Contribute to tender proposals and pitch new service models to prospective clients and funding bodies
Monitor market trends and competitor activity to inform strategic business development initiatives
Collect and analyse data on service performance and business development outcomes to drive continuous improvement
Support the training and supervision of care staff where required
What we're looking for
Demonstrated experience working within the NDIS environment, ideally in a care coordination or similar role
Strong understanding of NDIS principles, legislation, participant planning and service delivery requirements
Proven experience in business development, market analysis or service expansion within community services or aged care
Excellent interpersonal and communication skills with the ability to build relationships across diverse stakeholder groups
Strong organisational and time management capabilities, with the ability to prioritise multiple competing demands
Demonstrated problem-solving skills and the ability to work autonomously as well as collaboratively within a team
Proficiency with relevant IT systems and data management tools; experience with NDIS portals and databases is desirable
A current or willing to obtain relevant qualifications in disability support, aged care or community services
A valid driver's licence and access to reliable transport for client visits
A commitment to person-centred practice and the values of dignity, choice and empowerment
Working With Children Check (WWCC) and/or National Police Clearance, or willingness to obtain these as a condition of employment
