About the role

As a Care Partner, you'll work alongside older Australians to understand what's important to them and coordinate the supports that help them live independently and achieve their goals.

You'll build trusted relationships, complete assessments and reviews, develop care plans, manage individual budgets and make balanced decisions that consider participant choice, risk and available funding.

About you

You'll be someone who combines empathy with sound judgement.

You'll bring:

• Experience in aged care case management or care coordination

• Excellent communication and relationship-building skills

• Strong attention to detail

• Confidence managing budgets and balancing participant goals, risk and sustainability

• A collaborative approach and commitment to delivering high-quality, person-centred care
