About the opportunity:

• Provide quality client-focused care to patients, their families and carers, using a holistic approach.
• Assessment and monitoring of patients' physical and mental state.
• Be part of a multi-disciplinary team that requires you to provide highly responsive and flexible service to consumers and carers.

You will also be required to provide evidence of, or in the process of obtaining:

• A current NDIS clearance
• Certificate IV Lifestyle & Leisure
