About the role
Part-Time (4 days per week) – 12-Month Fixed-Term Contract (Maternity Leave Cover - Opportunity to Extend)
Westholme Support Coordination supports people with a disability to build their capacity by empowering them to make their own decisions. Using a person-centered approach, we focus on achieving participants goals and improving their quality of life, ensuring they are actively involved in their supports and can exercise choice and control.
Key responsibilities include:
Coordinate and implement participant NDIS plans in accordance with participant goals and preferences.
Assist participants to understand their NDIS funding and build capacity to manage their supports.
Identify suitable service providers and facilitate referrals.
Develop and maintain collaborative relationships with participants, families, carers, guardians and service providers.
Monitor participant progress and adjust supports where required.
Prepare reports for the NDIA, including change of circumstances, plan reassessments and progress reports.
Assist participants during plan reviews and advocate for appropriate supports where required.
Maintain accurate and contemporaneous case notes, records and participant documentation.
Monitor participant budgets and identify risks to plan utilisation.
Respond to participant enquiries in a timely and professional manner.
Liaise with government agencies, health professionals, allied health providers and community organisations.
Identify and respond appropriately to participant risks, incidents and safeguarding concerns.
Maintain participant confidentiality and privacy in accordance with relevant legislation.
Participate in team meetings, supervision and professional development activities.
Contribute to continuous improvement initiatives within the organisation.
Performance Expectations:
The successful applicant will be expected to:
Manage their allocated caseload independently.
Maintain accurate documentation within required timeframes.
Respond to participant communication promptly.
Meet organisational quality and compliance requirements.
Demonstrate professionalism, integrity and accountability.
Uphold the NDIS Code of Conduct and organisational policies.
Contribute positively to a participant-focused and collaborative workplace culture.
Working Arrangements:
Part-time position working four days per week.
Home-based role with flexible working arrangements.
Regular travel within the community to attend participant appointments and meetings.
Flexible scheduling to meet participant needs.
Laptop, mobile phone and required software provided.
Ongoing professional development and supervision provided.
Other Duties:
Conduct other duties as required.
Qualifications:
Certificate IV or higher in Community Services / disability work or Cert IV / Diploma in Case management and/or have extensive demonstrated experience in a Support Coordinator or similar role
