Position: Support Worker – Home Care

Salary: $36.08 per hour (Home Care Employee – Level 2, Pay Point 1)

About the Role

We are seeking caring, compassionate, and reliable Home Care Support Workers to join our team.

In this role, you will support individuals with disabilities in their homes and within the community, promoting independence, dignity, choice, and quality of life. Supports may include personal care, domestic assistance, medication administration, community access, attending medical appointments, and assistance with daily living activities.

Our participants have a diverse range of support needs, including mental health conditions, physical disabilities, complex behaviours, and mobility requirements. Applicants must be confident working independently, following individual support plans, and maintaining accurate documentation.

You will play an important role in helping participants live safely and comfortably at home, access their local community, build practical skills, and work towards their individual goals.

Key Responsibilities

• Provide personal care support while maintaining each participant’s dignity, privacy, and independence.
• Assist with mobility and manual handling tasks, including hoisting where required.
• Support participants with domestic duties, including cleaning, laundry, meal preparation, and general household tasks.
• Administer medication in accordance with medication plans, policies, and relevant training.
• Support participants to attend medical appointments, hydrotherapy sessions, shopping, recreational activities, and community outings.
• Provide support to participants experiencing mental health conditions or behaviours of concern.
• Follow behaviour support plans, restrictive practice requirements, and risk-management procedures.
• Complete accurate ABC reporting, shift notes, incident reports, medication documentation, and other required records.
• Encourage participants to develop independent living, communication, budgeting, meal-planning, and community-access skills.
• Build positive and professional relationships with participants, families, and other members of the support team.
• Work collaboratively with colleagues and management to provide consistent, person-centred support.
• Follow all NDIS Practice Standards, organisational policies, procedures, and workplace health and safety requirements.

Availability and Shift Requirements

Applicants must have broad availability and no significant restrictions on the hours they are able to work.

A range of shifts may be available, including:

• Morning shifts
• Afternoon and evening shifts
• Active night shifts
• Sleepover shifts
• Occasional weekend shifts

Applicants must be available Monday to Friday.

Key Requirements

• Previous experience in disability support, mental health, aged care, community services, or a related field is highly desirable.
• Certificate III or IV in Individual Support, Disability, Community Services, or a similar qualification is preferred.
• Confidence supporting participants with complex and varied support needs.
• Ability to work independently and as part of a team.
• Strong communication, observation, and interpersonal skills.
• A compassionate, reliable, and person-centred approach.
• Valid Australian driver’s licence and access to a reliable, insured vehicle.
• Current First Aid and CPR certificates, or willingness to obtain them.
• Paid NDIS Worker Screening Card, or willingness to obtain one.
• Willingness to complete required training and maintain all mandatory compliance documentation.
• Ability to meet the physical requirements of the role, including personal care and manual handling duties.
