About the role:
An exciting opportunity is available for an experienced Registered Psychiatric Nurse to join the Intensive Support Program (ISP) within Western Health’s Outer Community Mental Health Team.
This is a part-time position at 0.8 EFT, working Monday to Friday, providing the opportunity to maintain continuity with consumers while working within regular weekday hours.
The Outer Community Team provides specialist mental health care to consumers across the diverse and rapidly growing communities of Melton and Brimbank. You will join a supportive multidisciplinary team of nursing, occupational therapy, social work, psychology and medical clinicians who work collaboratively to improve outcomes for consumers and provide meaningful support to families and carers.
As an ISP Nurse, you will provide recovery-oriented, person-centred care to an allocated caseload of consumers with complex mental health needs. You will complete comprehensive assessments, coordinate care and develop individualised treatment and recovery plans in partnership with consumers, families, carers and the multidisciplinary team.
You will also contribute nursing-specific expertise and interventions to consumers across the broader service. ISP is a small, cohesive team where collaboration, shared clinical decision-making and an integrated approach to care are central to achieving positive outcomes.
