Registered Psychiatric Nurse - Intensive Support Program

About the role:

An exciting opportunity is available for an experienced Registered Psychiatric Nurse to join the Intensive Support Program (ISP) within Western Health’s Outer Community Mental Health Team.

This is a part-time position at 0.8 EFT, working Monday to Friday, providing the opportunity to maintain continuity with consumers while working within regular weekday hours.

The Outer Community Team provides specialist mental health care to consumers across the diverse and rapidly growing communities of Melton and Brimbank. You will join a supportive multidisciplinary team of nursing, occupational therapy, social work, psychology and medical clinicians who work collaboratively to improve outcomes for consumers and provide meaningful support to families and carers.

As an ISP Nurse, you will provide recovery-oriented, person-centred care to an allocated caseload of consumers with complex mental health needs. You will complete comprehensive assessments, coordinate care and develop individualised treatment and recovery plans in partnership with consumers, families, carers and the multidisciplinary team.

You will also contribute nursing-specific expertise and interventions to consumers across the broader service. ISP is a small, cohesive team where collaboration, shared clinical decision-making and an integrated approach to care are central to achieving positive outcomes.

Key responsibilities:

• Providing recovery-oriented assessment, intervention and care coordination.
• Developing and reviewing individualised care plans with consumers, families and carers.
• Working collaboratively with psychiatrists, allied health clinicians and external service providers.
• Supporting safe and coordinated transitions between services.
• Providing nursing-specific assessment, intervention and consultation.
• Maintaining accurate and timely clinical documentation.
• Contributing to clinical governance, quality improvement and service development.
• Practising in accordance with the Mental Health and Wellbeing Act 2022 and relevant professional and organisational standards.
• Working within relevant clinical and service-delivery key performance indicators.

About you:

To be successful in this role, you will have:

• Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
• A Bachelor’s degree in Psychiatric/Mental Health Nursing or equivalent, or a Bachelor’s degree in Nursing with a postgraduate qualification in psychiatric or mental health nursing.
• A minimum of two years’ experience in community mental health or a similar clinical setting.
• Experience supporting consumers with complex mental health needs.
• Knowledge of recovery-oriented practice and relevant mental health legislation.
• Strong clinical assessment, care coordination and decision-making skills.
• Well-developed communication and interpersonal skills.
• The ability to work effectively within a multidisciplinary team.
• A current Victorian driver’s licence and the ability to drive Western Health vehicles.
