Help young people build life beyond care.
Every young person deserves the opportunity to thrive. As a Case Manager – Next Step Plus, you'll support young people transitioning from out-of-home care to build the confidence, skills and connections they need to live independently and achieve their goals.
Working across the North Coast region, you'll manage your own caseload while delivering outreach-based, strengths-focused support that empowers young people to navigate housing, education, employment, health, wellbeing, relationships and independent living.

About the Role
As a Case Manager, you'll work alongside young people aged 15 years and over who are preparing to leave care, as well as young adults who have already exited care.
You'll provide outreach-based case management that is practical, collaborative and tailored to each young person's strengths, aspirations and individual needs. Your role will focus on helping young people achieve sustainable outcomes across:
You'll coordinate services, advocate for young people, develop goal-focused plans, and work closely with Child Safety, community organisations and other stakeholders to ensure young people have the support they need to thrive into adulthood.
This role requires regular travel throughout the service area and offers the opportunity to work independently while remaining well connected to a supportive and experienced team.
