About the role

Working within the Care team, you’ll collaborate directly with participants, families and stakeholders to deliver high-quality assessments, personalised care planning and comprehensive case management, which promotes participants goals and wellness outcomes.

Key responsibilities:

• Conduct HCP assessments and reviews in participants’ homes

• Convert client needs into funding opportunities to maximise package utilisation

• Build strong relationships with participants, support networks and internal teams

• Deliver high-quality, person-centred care

• Provide comprehensive case management and advocacy

• Collaborate with allied health, nursing partners and others to coordinate multidisciplinary supports

• Manage incidents, feedback and complaints in line with ACQS requirements

• Record and maintain high quality, comprehensive documentation and case notes

• Travel is required with km reimbursement provided

About you

• Qualifications in case management (e.g. Diploma in Nursing, Community Services, Human Services, Ageing or Disability), with ideally 3+ years industry experience

• Ability to anticipate risk, evaluate impacts and make informed, balanced, confident decisions

• A natural influencer who engages stakeholders and drives alignment and action

• Highly organised, agile and adept at managing competing priorities

• Understands the Strengthened Aged Care Quality Standards, navigating funding and clinical escalation pathways

• Committed to continuous improvement and service excellence

• Balances care outcomes with financial sustainability
