About the role
Working within the Care team, you’ll collaborate directly with participants, families and stakeholders to deliver high-quality assessments, personalised care planning and comprehensive case management, which promotes participants goals and wellness outcomes.
Key responsibilities:
Conduct HCP assessments and reviews in participants’ homes
Convert client needs into funding opportunities to maximise package utilisation
Build strong relationships with participants, support networks and internal teams
Deliver high-quality, person-centred care
Provide comprehensive case management and advocacy
Collaborate with allied health, nursing partners and others to coordinate multidisciplinary supports
Manage incidents, feedback and complaints in line with ACQS requirements
Record and maintain high quality, comprehensive documentation and case notes
Travel is required with km reimbursement provided
About you
Qualifications in case management (e.g. Diploma in Nursing, Community Services, Human Services, Ageing or Disability), with ideally 3+ years industry experience
Ability to anticipate risk, evaluate impacts and make informed, balanced, confident decisions
A natural influencer who engages stakeholders and drives alignment and action
Highly organised, agile and adept at managing competing priorities
Understands the Strengthened Aged Care Quality Standards, navigating funding and clinical escalation pathways
Committed to continuous improvement and service excellence
Balances care outcomes with financial sustainability
