Role Responsibilities:

• Coordinate formal and informal supports

• Manage clients with complex needs and handle crisis situations

• Collaborate with a Multidisciplinary Panel

• Engage with Mental Health, Justice System, Housing, and Health sectors

• Undertake clerical administration and report writing

• Network and communicate with key stakeholders

• Work independently

• Work in a sensitive, complex and demanding environment with a high degree of autonomy and self-management.

Specialist Support Coordinator (SSC) specific responsibilities:

• Develop and implement support plans for participants with high-level risks or barriers
• Conduct detailed risk assessments and implement mitigation strategies
• Stabilise crisis situations and coordinate urgent, intensive supports
• Navigate complex service systems including justice, homelessness, and hospital settings
• Collaborate with statutory agencies, guardians, and tribunals in high-risk cases
• Prepare evidence-based reports for complex funding requests
• Participate in NDIA Complex Support Needs Panel reviews and case conferences

Qualifications & Experience:

• Relevant qualification in the Human Service Sector or a minimum of 3 years’ experience in a similar disability sector role

• Preferred: Degree in Social Work, Psychology, Occupational Therapy, Counselling, Human Services, or Disability Studies

• Experience in Forensic Disability or Forensic Mental Health is a plus

• Demonstrated in-depth knowledge of the NDIS and experience working with its participants

• Valid Driver’s License with a suitable, insured vehicle

• A current Working with Vulnerable People registration

• Recent police check (or willingness to obtain one)
