Case Manager - Low-Cost Accommodation Support Program

Merri Outreach Support Service (MOSS) has been supporting people experiencing or at risk of homelessness since 1989. Guided by our vision of striving to end homelessness, sustaining housing and strengthening communities, we help people access housing, support services and opportunities that improve wellbeing and social inclusion. Through our Support and Community Engagement Program, we work alongside older people experiencing homelessness, housing instability, social isolation and complex needs, helping them remain connected, independent and safely housed.

We are seeking a passionate and skilled Case Manager – Low-Cost Accommodation Support Program to join our team. This rewarding role supports older people and vulnerable adults living in low-cost accommodation and public housing who may be experiencing homelessness, social isolation, mental health challenges, chronic health conditions or other barriers to accessing services. 

Working across sites in Northcote and Brunswick, you will provide assertive outreach, case management, housing advocacy, tenancy support, referrals and practical assistance to help people improve housing stability, wellbeing and community connection. You will build trusting relationships with clients, work collaboratively with partner agencies and contribute to positive outcomes for some of the community's most vulnerable people. 

We are looking for someone with experience in community services, homelessness, housing, aged care or a related field who is committed to person-centred practice and social justice. You will have strong communication and advocacy skills, an ability to engage respectfully with people experiencing complex challenges, and a genuine commitment to supporting diverse communities. A relevant qualification in Community Services, Social Work, Community Development, Welfare Studies or a related discipline is highly desirable.
