Divert Case Manager

We are seeking multiple passionate and client-focused professionals to support individuals and families who are experiencing homelessness or are at risk of entering the homelessness system.

As our Divert Case Manager, you will play a vital role in helping people navigate housing crises, access emergency accommodation, and connect with services that support long-term housing stability and wellbeing. Working from a person-centred, strengths-based and trauma-informed approach, you will support clients to develop individual plans and achieve sustainable outcomes.

Working closely with the Toward Home Alliance, Lutheran Care teams and external stakeholders, you will contribute to a collaborative, integrated service model focused on reducing homelessness and ensuring people receive the right support at the right time. 

• Provide high-quality, client-centred case management support to individuals and families experiencing, or at risk of, homelessness.
• Undertake assessments and develop Individual Support Plans that identify goals, support needs and housing pathways.
• Support clients to secure emergency accommodation and work towards sustainable long-term housing outcomes.
• Provide advocacy, referrals and practical support to address issues impacting housing stability, including mental health, substance use, physical health and life skills.
• Build strong therapeutic relationships with clients through responsive and timely engagement.
• Maintain accurate case notes, client records and data management within relevant systems, including H2H.
• Collaborate with Toward Home Alliance partners and community services to coordinate holistic support for clients.
• Manage a diverse caseload, prioritising responses for clients with complex and multiple needs.
• Participate in supervision, reflective practice, professional development and continuous improvement activities.
• Support phone and face-to-face triage services across Lutheran Care and Alliance partner locations as required.
• Utilise digital tools and systems to create accessible client profiles and support coordinated service responses.

• Relevant qualifications or specialist experience in Community Services, Social Work, Psychology or a related field.
• Demonstrated experience in person-centred case management, assessment and referral using a trauma-informed approach.
• Knowledge and understanding of homelessness and the challenges faced by people experiencing housing instability.
• Understanding of rapid rehousing and Housing First models.
• Strong interpersonal skills with the ability to engage effectively with clients, colleagues and stakeholders.
• Excellent written and verbal communication skills.
• Proven ability to manage competing priorities, organise work effectively and achieve outcomes.
• Ability to work independently, use initiative and contribute positively to a team environment.
• Commitment to continuous improvement and quality service delivery.
• Competence using databases, Microsoft Office applications and online communication platforms.

• Knowledge of the impacts of trauma and abuse in relation to homelessness.
• Experience or training in the Australian Homelessness Vulnerability Triage Tool (AHVTT/VI-SPDAT).
• Experience working with Aboriginal and Torres Strait Islander peoples and culturally and linguistically diverse communities.
• Knowledge of domestic and family violence and its impact on individuals and families.
• Safety planning experience.
• Experience using the H2H database.
