Dietitian

Amity Health is seeking a passionate and client-focused Dietitian to join our Integrated Chronic Disease Care (ICDC) team, supporting individuals across regional Western Australia to improve their health, wellbeing and quality of life.  

In this rewarding role, you will deliver evidence-based nutrition and dietetic services to individuals and groups living with chronic health conditions. Working as part of a multidisciplinary team, you will support clients to develop the knowledge, skills and confidence to better manage their health and achieve meaningful, sustainable outcomes.  

About Integrated Chronic Disease Care (ICDC) 

Amity Health delivers the Integrated Chronic Disease Care (ICDC) program on behalf of the WA Primary Health Alliance across the Great Southern, Wheatbelt and South Coastal Goldfields regions. The program supports adults aged 18 years and over who are living with chronic conditions, including cardiovascular disease, diabetes, respiratory disease and morbid obesity, and who require additional care coordination to improve their health outcomes.  

The broader Allied Health ICDC team provides coordinated, evidence-based care through a multidisciplinary approach that supports improved health and wellbeing outcomes and strengthens clients’ capacity to self-manage their conditions. Services are delivered through both in-person and telehealth appointments to support access across regional communities 

About the Role 

This is an excellent opportunity for a Dietitian who enjoys building genuine relationships with clients and supporting positive behaviour change through practical, person-centred care. 

As a key member of the ICDC team, you will work closely with clients, General Practitioners and allied health professionals to deliver coordinated care that supports improved nutrition, self-management and long-term health outcomes.  

As the Dietitian, you will: 

• Deliver high-quality, evidence-based dietetic services to individuals and groups across the Coastal Wheatbelt and Telehealth regions.  
• Conduct comprehensive nutritional assessments and develop tailored care plans and therapeutic interventions.  
• Support clients living with chronic conditions to improve their nutrition, health and wellbeing outcomes.  
• Contribute to coordinated care planning and service delivery as part of a multidisciplinary team.  
• Design and facilitate group education sessions that encourage behaviour change and support self-management.  
• Participate in preventative health programs and community health promotion activities.  
• Work collaboratively with General Practitioners and other healthcare providers to support integrated care planning and referral pathways. 
• Act as a clinical resource for best-practice dietetic management within a multidisciplinary environment.  
• Maintain accurate clinical records and contribute to reporting, evaluation and service improvement activities.  

This is a Part-Time role, 2 days per week, based in Moora, WA.  

About You 

You are a compassionate and motivated Dietitian who values person-centred care and is committed to supporting people living with chronic health conditions to achieve meaningful health outcomes. 

You enjoy working collaboratively, building strong professional relationships and delivering practical, evidence-based interventions that empower individuals to better manage their health. 

You bring: 

• Experience applying evidence-based dietetic interventions within a primary healthcare or community health setting.  
• Strong written, verbal and interpersonal communication skills.  
• The ability to work collaboratively and effectively within a multidisciplinary team.  
• Excellent organisational and time management skills, including the ability to manage a clinical caseload and competing priorities. 
• Confidence using electronic client management systems and standard office applications.  
• A commitment to professional excellence, evidence-based practice and positive client outcomes.  

Essential Requirements 

• Tertiary qualification in Nutrition and Dietetics, Human Nutrition or Nutrition Science.  
• Current Accredited Practising Dietitian (APD) credential with Dietitians Australia, or eligibility to obtain and maintain.  
• Current National Police Clearance.  
• Current WA Driver’s Licence.  

Desirable 

• Postgraduate qualifications in Clinical Nutrition, Public Health, Chronic Disease Management, Health Promotion, or a related field.  
• Experience working with clients in rural and regional communities.  
• Experience working collaboratively with General Practitioners in client assessment, care planning and case conferencing.  
• Experience using electronic patient records and clinical database systems.  
• A demonstrated commitment to ongoing professional development and reflective practice.
