Clinical Case Manager – Community (Registered Nurse) | Inner West Sydney

• Conduct comprehensive nursing assessments and develop tailored care plans
  
• Provide clinical oversight and support to care staff and allied health partners
  
• Monitor care outcomes and adjust plans to meet changing needs
  
• Build strong, trusting relationships with clients and their families

Requirements

• Current AHPRA registration as a Registered Nurse
  
• Demonstrated experience in community or aged care settings
  
• Excellent communication and time management skills
  
• Ability to think critically and respond effectively in dynamic situations
  
• Current driver’s licence and reliable, insured vehicle
  
• National Police Check and NDIS Worker Screening Check (or ability to obtain)
