Co Care Partner - Hybrid

We are looking for an experienced and compassionate Co-Care Partner to coordinate high-quality, person-centred home care services for our clients. This role is office based in our NSW Parrmatta office - Working hours are 9am - 5pm.

What you will do

This is a varied, hands-on role for someone who understands government-funded home care, builds trusted relationships and takes genuine ownership of the client experience. You will work closely with clients, families, care professionals and service providers to ensure each client receives responsive, safe and well-coordinated support. this role

Role responsibilities:

• Develop, implement and regularly review person-centred support plans.
• Coordinate services aligned with each client’s needs, goals, preferences and available budget.
• Conduct scheduled client meetings, regular check-ins and formal care reviews.
• Build strong relationships with clients, representatives, providers and health professionals.
• Identify and escalate incidents, safeguarding concerns and changes in client risk.
• Maintain accurate assessments, support plans, case notes, consent records and service documentation.

Registered Nurses and Enrolled Nurses may also undertake clinical assessments, reviews and high-risk client monitoring within their qualifications and scope of practice.

What you’ll bring

• At least two years’ experience in aged care, care coordination, client services or a similar regulated service environment.
• Experience coordinating government-funded home care services, individualised budgets or self-managed care arrangements.
• Sound knowledge of aged care quality requirements, client rights, safeguarding and risk escalation.
• Strong communication skills and the confidence to manage sensitive conversations.
• Excellent organisation, judgement and problem-solving skills.
• A proactive approach and the ability to take ownership of matters through to resolution.
• A qualification in aged care, community services, disability, social work, nursing, allied health or a related field is highly regarded.
