About the role
Make a meaningful difference to the health and wellbeing of young people experiencing or at risk of homelessness.
As a Community Health Nurse, you'll provide vital primary health care, outreach support and health education, working alongside a multidisciplinary network of services to improve access to care and create positive health outcomes for young people across the Barwon and Colac regions.
