About the role
About The Role
Angels Care is seeking an experienced Support Coordinator to join our growing team. In this pivotal role, you will work directly with NDIS participants and their families to implement support plans, build capacity, and coordinate a network of services that genuinely reflect participant goals and choice.
You will manage a caseload of participants across a range of disability types, including physical disability, intellectual disability, acquired brain injury, and psychosocial disability. You will work collaboratively with allied health professionals, community services, and government agencies to ensure seamless, participant-centred outcomes.
This is a full-time position based in Western Sydney with some hybrid flexibility. A vehicle and willingness to travel within the region is required.
Key Responsibilities
Develop and maintain strong, trust-based relationships with NDIS participants and their support networks Implement NDIS plans and link participants with appropriate funded and mainstream supports Build participant and carer capacity to understand and use NDIS funding effectively Monitor support plan budgets and prepare NDIS progress reports Liaise with Local Area Coordinators, planners, and the NDIA as required Maintain accurate case notes and documentation in compliance with NDIS Practice Standards Participate in service reviews, case conferences, and multi-disciplinary meetings Support participants through plan reviews and goal-setting processes
Required Qualifications & Checks
Minimum 2 years' experience in Support Coordination or a related disability/community services role Solid understanding of the NDIS, including plan categories, funding types, and participant rights Current NDIS Worker Screening Check NSW Working with Children Check (WWCC) Valid Australian Driver's Licence and reliable vehicle Proficiency with NDIS management systems and digital case noting tools Right to work in Australia
Preferred Skills & Experience
Tertiary qualification in Social Work, Disability Studies, Community Services, or a related field Experience with Specialist Support Coordination for participants with complex needs Familiarity with PACE (the NDIA's participant and provider system) Experience using CRM or practice management software
