Our client is a dynamic, progressive and experienced not-for-profit aged care provider in Brisbane, offering in-home services including Support at Home and CHSP, respite, transport and aged accommodation for a culturally diverse ageing population. Their services are delivered by a highly trained, professional and caring team, working collaboratively with other health professionals to ensure the right care at the right time.

The primary purpose of this position is to build trusting relationships with clients and their families, and to deliver compliant, person-centred care management that promotes independence, wellness and reablement so clients can confidently live at home. Due to growth, our client is seeking two experienced Care Partners to join their team. This is a 9–5, office-based role with a requirement to drive to client homes.

Reporting directly to the CFO, some of your responsibilities will include:

• Conducting holistic assessments, care planning and reviews in clients' homes
• Managing new client intake, onboarding and service agreements
• Developing and maintaining care plans, budgets and documentation
• Rostering best-matched staff and coordinating internal teams and external providers
• Monitoring budgets and compliance in line with the Aged Care Quality Standards

The successful candidate will have:

• An aged care qualification with demonstrated experience in a similar home care role
• Current First Aid and CPR, plus a National Police Check
• A valid Australian driver's licence and full Australian work rights
• Strong client relationship-building, communication and stakeholder skills
• Proficiency in the My Aged Care Portal, care databases and MS Office

• $85,000 – $95,000 + Super — Negotiable Based on Experience
