What you'll be doing

The Complex Patient Care (CPC) Navigator provides specialised services for older individuals and vulnerable persons in the inpatient settings with complex care and health needs, including patients with challenging behaviours. The CPC Navigator offers a person-centred approach, focusing on comprehensive assessment, care planning, and coordination to ensure the timely and appropriate discharge of these patients.
The CPC Navigator ensures efficient and effective co-ordination from admission to discharge of patients with the aim of effecting an improved patient experience as well as access and flow within the hospital. This role works in conjunction with the Deputy Director of Nursing and Midwifery (DDoNM) to co-ordinate resources that operationally impact on patient flow from patient admission to discharge and institutes practices aimed at improving discharge times and processes optimising bed availability.
The CPC Navigator also advocates for older and vulnerable persons and their families/ carers while balancing competing/ conflicting priorities and must communicate effectively with families and external service providers to achieve the best outcomes. They make critical decisions regarding clinical care, assessment, case management, and discharge planning, with support from the DDONM and/or Director of Medical Services.
Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife or qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award.

Demonstrated competence in the use of health-related information technology.
