About the RoleJoin the Pakenham Community Mental Health Team as a Grade 3 Social Worker Case Manager and make a meaningful difference in the lives of adults experiencing complex mental health challenges. This is a full time, fixed term position until March 2027 but we are happy to take part time applications also.
Working within a collaborative multidisciplinary team, you'll provide recovery-oriented, person-centred care that empowers consumers to achieve their goals and live well in the community. This is a varied role where no two days are the same, combining clinical expertise with strong therapeutic relationships and community engagement. In this role, you will:
The Pakenham Community Mental Health Team is part of Casey Area Mental Health Service, supporting adults across Melbourne's rapidly growing south-east corridor.
Our team includes Social Workers, Occupational Therapists, Psychologists, Nurses and Psychiatrists who share a commitment to recovery, inclusion and person-centred care. With a supportive leadership team and strong culture of collaboration, you'll be encouraged to bring your ideas, continue developing your practice and genuinely make an impact.
