Join the Pakenham Community Mental Health Team as a Grade 3 Social Worker Case Manager and make a meaningful difference in the lives of adults experiencing complex mental health challenges. This is a full time, fixed term position until March 2027 but we are happy to take part time applications also.

Working within a collaborative multidisciplinary team, you'll provide recovery-oriented, person-centred care that empowers consumers to achieve their goals and live well in the community. This is a varied role where no two days are the same, combining clinical expertise with strong therapeutic relationships and community engagement. In this role, you will:

• • Mannage a diverse caseload using recovery-focused case management.
  • Complete comprehensive psychosocial and mental health assessments.
  • Provide outreach support in consumers' homes and community settings.
  • Participate in duty, crisis response and multidisciplinary care planning.
  • Collaborate with families, carers and external agencies to achieve positive outcomes.

The Pakenham Community Mental Health Team is part of Casey Area Mental Health Service, supporting adults across Melbourne's rapidly growing south-east corridor.

Our team includes Social Workers, Occupational Therapists, Psychologists, Nurses and Psychiatrists who share a commitment to recovery, inclusion and person-centred care. You are an experienced mental health Social Worker who values compassionate, evidence-based care and thrives in a collaborative team environment. To succeed in this role, you'll have:

• A recognised Social Work qualification and eligibility for AASW membership.
• Experience working within adult mental health or a related clinical setting.
• Well-developed psychosocial assessment, risk assessment and case management skills.
• Knowledge of Victorian mental health legislation and recovery-oriented practice.
