NDIS Support Coordinator (level 2 or level 3)

Job Type: Permanent position | Full time position

• Conduct comprehensive participant assessments to identify individual strengths, goals, risks, and support requirements.
• Review NDIS plans and develop practical support coordination strategies tailored to participant needs.
• Manage an allocated caseload of participants requiring ongoing coordination of supports.
• Coordinate disability, health, mental health, housing, education, employment, and community services to ensure integrated service delivery.
• Undertake risk assessments relating to participant wellbeing, service access, housing stability, health, safeguarding, and support needs.
• Identify barriers affecting implementation of participants’ NDIS plans and develop practical strategies to minimise risks and improve outcomes.
• Monitor participant progress and review support arrangements as circumstances change.
• Advocate on behalf of participants to facilitate equitable access to services and supports.
• Assist participants during NDIS plan reviews by coordinating reports and supporting evidence.
• Build and maintain professional working relationships with families, carers, guardians, allied health professionals, hospitals, medical practitioners, local councils, government departments, and community organisations.
• Participate in multidisciplinary meetings and case discussions to achieve coordinated outcomes.
• Maintain accurate case notes, assessments, progress reports, and participant records in accordance with organisational policies and NDIS Practice Standards.
• Ensure compliance with the NDIS Act, Practice Standards, Code of Conduct, Quality and Safeguards requirements, and relevant legislation.

About You You will possess:

• 1 year of relevant post qualification work experience
• Demonstrated experience in case management, support coordination, care coordination, social work, community services, or disability services.
• Strong assessment, analytical, problem-solving, and decision-making skills.
• Experience coordinating complex services across multiple agencies and professional disciplines.
• Excellent communication, negotiation, and stakeholder engagement skills.
• Sound understanding of person-centred, strengths-based, and recovery-oriented practice.
• Ability to work independently while managing competing priorities and maintaining high professional standards.
• High-level written communication and report writing skills.

Essential Qualifications Applicants must hold one of the following:

• Bachelor or Master’s degree in Social

Work

• Human Services
• Community Services
• Psychology
• Occupational Therapy
• Nursing
• Behavioural Science
• Disability Studies
• or another closely related discipline

Essential Requirements

• Current National Police Check
• NDIS Worker Screening Check
• Working with Children Check (where applicable)
• Current Australian Driver Licence
• Full Australian working rights
