We are currently seeking experienced Support Workers to join our growing team across the Perth metropolitan region. We have a range of shifts available, including morning, afternoon, active night and sleepover shifts. Please indicate your availability and preferred shift type in your application.

Key Responsibilities

• Support participants to work towards achieving their individual goals in line with their support plans
• Assist with daily living activities, including personal care, cooking, cleaning, shopping and community access
• Support participants to build confidence, independence and life skills
• Assist with medication administration in line with organisational policies and training
• Implement positive behaviour support strategies
• Encourage participation in age-appropriate activities, employment and social engagement
• Maintain a safe, respectful and supportive environment at all times
• Monitor participant wellbeing and report any concerns to the Manager
• Accurately complete required documentation, progress notes and communication records
• Participate in individual planning processes and monitor outcomes
• Liaise with families, carers, case managers, allied health professionals and community agencies
• Work in line with NDIS standards, WHS guidelines, privacy and confidentiality requirements
• Collaborate with team members to ensure consistent and high-quality service delivery

Skills & Experience

• Qualification preferably in community/disability/individual support/aged care/mental health and/or relevant experience
• Experience working with clients with challenging behaviours
• Well-developed verbal and written communication skills
• Current Driver Licence and reliable vehicle
• 100-point ID check
• Valid visa/working rights in Australia

Mon to Sun- 6AM to 2PM & 2PM to 10 PM
