About the role
Manning Support Services (MSS) is an award-winning community service organisation that has been operating on the NSW Mid North Coast for more than 40 years. MSS is a multi-service not-for-profit, with a mission to enhance the quality of people’s lives by providing services which enable them to live with dignity and respect.
MSS has an opportunity for a full-time Care Partner to join our Home Care team in Taree. The position is 38 hours per week, Monday to Friday, with the option of a 9-day fortnight.
At MSS, we believe strong relationships are at the heart of great care. Our Care Partners take the time to understand each client — what is important to them, what they want to achieve and how they want to live their life.
This is a case management and care coordination role, rather than a direct support role. Care Partners work alongside clients to plan, coordinate and review the services and supports that help them achieve their goals. This includes care planning, service coordination, budget management and responding to changing needs, ensuring supports continue to reflect each client’s choices, preferences and circumstances.
Care Partners are a trusted point of contact throughout the client’s care journey. Through regular communication, home visits and reviews, they monitor wellbeing, identify and manage risks, coordinate with service providers and respond when circumstances change.
Tasks and responsibilities include:
Care planning and coordination for Support at Home and Commonwealth Home Support Program clients
Management of client budgets and service delivery
Ongoing monitoring of care, including in home assessment and reviews
Assessing and managing risk in client homes
Building strong, respectful relationships through clear and effective communication
Managing client feedback and incidents
Ensuring services meet the strengthened Aged Care Quality Standards, providing person-centred care, supporting client choice, maintaining accurate documentation, and safeguarding client rights
Essential Qualifications and Requirements:
Certificate III or IV in Community Services, Aged Care, Individual Support, or similar, or relevant work experience
Experience supporting older people in aged care, health, or disability services
Knowledge of aged care programs, standards, and the rights of older people
Ability to use client record or care management systems (training provided)
National Criminal History Check, or willingness to obtain one at MSS's expense
Provisional or full NSW driver’s licence
Access to a reliable, registered vehicle with at least third-party property damage insurance
Skills & Experience
Experience delivering person-centred, goal-focused care and support.
Strong assessment, care planning, budget management, and problem-solving skills.
Proficient in Microsoft Office, client management systems, and My Aged Care.
Excellent communication skills with the ability to build relationships with clients, families, and service providers.
Experience working with diverse communities and coordinating service referrals.
Understanding of workplace health and safety practices.
Compassionate, professional, and committed to client choice and independence.
Strong interpersonal skills with a focus on trust, privacy, and confidentiality.
Well-organised, adaptable, and able to work both independently and collaboratively.
Resilient with excellent time management and prioritisation skills.
