Pathways and Transition Support Officer

We are seeking to establish a pool of Pathways and Transition Support Officers for permanent and fixed term, full-time and part-time vacancies which may arise during 2026 and up to the end of the 2027 school year. Appointments may be offered at any time for vacancies that commence during this period.

Newton Moore Education Support Centre (ESC) is an Independent Public School nestled in the thriving and idyllic coastal City of Bunbury, WA. The Centre is specifically designed to meet the needs of Years 7 to 12 students with intellectual disabilities or autism with high needs. The Role

The Pathways and Transition Support Officer plays a key role in supporting students with disability to make successful transitions from school to employment, further training and meaningful participation in the community. Working collaboratively with students, families, school staff, feeder schools, employers, Registered Training Organisations (RTOs) and community agencies, you will help identify students' aspirations, strengths and interests and assist in the development and implementation of individual post-school pathway plans.

Suitable applicants will ideally have experience working with young people with disability and an understanding of transition planning, employment pathways and community engagement opportunities. You will ideally be skilled in establishing and maintaining effective partnerships and networks that support positive outcomes for students and ensure they are connected to appropriate services, training providers and employment opportunities.

Suitable applicants should ideally possess highly developed communication and interpersonal skills and the ability to work effectively with students, families, staff and external agencies. In this role, you will assist in coordinating student transition programs, support assessments and pathway planning processes, maintain accurate case notes and reports, facilitate student engagement activities, and provide advice and support to staff on strategies that enhance student outcomes.

You will also contribute to whole-school initiatives and assist the leadership team with crisis management and response activities when required.
