Job Description:

We are seeking a dedicated Physical Therapist to join our Home Health team. In this role, you will be responsible for providing physical therapy services to patients in their homes, improving their physical and cognitive functioning, and ensuring enhanced independence. You will play a crucial role in patient and caregiver education and will be instrumental in the continuous monitoring and reporting of patient progress.

Primary Duties and Responsibilities:

- Conduct physical therapy assessments and formulate treatment programs tailored to each patient’s needs.

- Document and communicate patient reactions to treatments and any changes in their conditions.

- Educate patients and caregivers about therapy programs and the correct use of medical devices.

- Supervise physical therapy assistants and, occasionally, certified nursing assistants.

- Contribute to team and agency development through the preparation of detailed reports and participation in staff meetings.

Qualifications:

- Must be a licensed Physical Therapist in Pennsylvania.

- Experience in Home Health, Hospital, LTC, or SNF settings preferred.

- Must not have a primary background in Pediatrics (as per job requirements).

- Valid Pennsylvania state driver’s license required.

- Completion of a Master’s degree in Physical Therapy.

Travel Requirements:

- Local travel required.

Schedule:

- Monday to Friday, 8:00 AM - 5:00 PM

- Includes 1 weekend per month and 2 holidays per year

Compensation Details:

- $10,000 signing bonus available.
