Integrated Care Unit Manager

Provide strong leadership and senior management of multiple sites and/or services whilst overseeing the operation of the Integrated Care Unit to ensure delivery of an integrated and effective service.  

The Integrated Care Unit provides services across SESLHD that support people with chronic health conditions to live well in the community. The Unit Manager supports their teams to deliver seamless, client centred care that addresses both health and social care needs. The Unit delivers a suite of services providing individualised, goal-centred care coordination, health care navigation and health coaching to increase clients’ health literacy and enable condition self-management. The Unit delivers culturally safe health services for Aboriginal clients. 
The Integrated Care Unit includes the following services:

• Planned Care for Better Health (PCBH)
• ED to Community
• South East Aboriginal Health Care (SEAHC)
• 48 Hour Follow Up (for Aboriginal clients discharged from Hospital)
• Out of Hospital Care (OHC) (ComPacks, Safe and Supported at Home (SASH) and End of Life care) – including responsibilities as OHC Relationship Manager
• Aboriginal Cancer Care Coordinator
• Aboriginal Supportive and Community Care Coordinator 
• Bulbuwil Aboriginal Healthy Lifestyle program (Sutherland & Georges River)
• Legal Advice Service (Sutherland, St George and Calvary Hospitals and community health)

Selection Criteria 

1. Relevant tertiary qualifications in a health discipline, Public Health or Health Management or relevant equivalent work experience, or a combination of study and work experience
2. Proven experience in senior leadership and management in a large and complex organisation
3. Demonstrated experience of implementing change to meet strategic outcomes
4. Demonstrated ability in management of human resources/financial/administrative/and/or other services
5. Excellent strategic planning and policy development skills, including the ability to make complex judgements and take initiative within the delegated area
6. Excellent negotiation, decision making and influencing skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, through all organisational levels
7. Highly developed and effective written and oral communication skills
8. A willingness to travel in accordance with the demands of the position
