Functions of the Role 

The Care Partner provides valuable support to clients under the Commonwealth Home Support Program (CHSP) and Support at Home Program (S@H) or Queensland Community Support Scheme (QCSS) and National Disability Insurance Scheme (NDIS) funding streams ensuring they receive high quality care services. 

Duties and Tasks (a complete list is able to be found on the Position Description)

• Ensures that clients are partnered to actively participate in their care to maintain their independence;
• Ensures that the handling and storage of client information and records is in line with organisational standards, confidentiality and other organisational policy and commercial-inconfidence procedures;
• Acts as a client advocate with other service providers when required;
• Promotes the organisation's services and gains commitment from prospective clients while making recommendations that will best benefit the client;
• Responsible for liaising with the Services Team to ensure optimal service delivery to all clients by partnering with them through their journey from admission through to discharge.

Key Selection Criteria

• Minimum qualification of a Certificate III in Aged Care, Disability or relevant discipline or Cert IV qualification in line with funding requirements for providing government funded aged care and disability services or a related field;
• Ability to meet the requirements of Criminal History screening procedures;
• 2 years' experience in the industry preferred;
• Sound understanding of the principles of Consumer Directed Care.

MUST HAVES for this role:

• Correct qualifications;
• Support at home experience;
• High level of communication skills.

NICE TO HAVES for this role:

• Case management experience;
• Sound understanding of consumer directed care;
• Excellent time management  skills.
