About The Role
You will be based at Concord Repatriation General Hospital, which has provided outstanding healthcare for more than 70 years. Building on its proud heritage of caring for the veteran community, the hospital now delivers a broad range of specialist and general health services to the communities of Concord, Strathfield, Burwood and surrounding areas.
You will be providing palliative care support and education to health care professionals across the facility campus and facilitate best practice clinical care to individual patient palliation and family/carer support. Which includes:
Complex symptom management - Works closely with specialist palliative care registrars, JMO and consultants in identifying physical, psychological, social, and spiritual needs of patients. Assess and recommend non-pharmacological management of symptoms to patients receiving palliative care.Multidisciplinary collaboration -Attend ward rounds, multidisciplinary meetings and family meetings.Care coordination -Liaise with the treating team, ward nurses, allied health, community palliative care services, and residential aged care team to ensure continuity of care. Organises palliative care service referrals outside of health district catchment area. Guides other acute care team in organising appropriate palliative care follow-up.Family support and discharge planning -Provide education, counselling and emotional support to patients and families, including preparation for deterioration and end-of-life care. Coordinate discharge home for end-of-life care in the community.Documentation -Complete comprehensive consultation notes, symptom assessments, care plans, and handovers in the electronic medical record. Education -Teach ward nurses, junior doctors and allied health staff on symptom management, syringe drivers, communication, and end-of-life care. Clinical leadership -Act as a resource person for complex palliative care issues across the hospital.Quality improvement -Participate in audits, guideline implementation, service improvement projects, and research activities.

Ideal Candidate
Current registration with AHPRA as a Registered Nurse (Division 1) with relevant post-registration qualifications and at least 3 years experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.Demonstrated knowledge and expertise in specialist Palliative Care clinical practices including assessment, planning, treatment management, and clinical leadership underpinned by evidenced based practice.
