You will be working in the Aged Services Emergency Team (ASET), which provides a specialist consultative service for patients aged 65 years and over, or 50 years and older for Aboriginal and Torres Strait Islander people who present to the Royal North Shore Hospital (RNSH) Emergency Department (ED). ASET may additionally see other age groups with frailty or pre-frailty.
As part of the Aged Care Department, ASET works collaboratively with ED staff, community services and inpatient teams to support older and frail patients to receive appropriate care beyond ED. Their aim is not only to improve the health outcomes of older people but also to minimise the requirement to remain in hospital and to prevent readmissions once they are discharged. This includes facilitating discharge home with services in place or supporting admission with a care plan informed by their preferences and degree of frailty. ASET plays a critical role in improving patient experience, enhancing clinical outcomes, and supporting system efficiency.
