MiCare is a not-for-profit organisation that exists to enable people from diverse backgrounds to lead dignified and meaningful lives. We offer aged care, home care, retirement living and migrant support services, with a strong focus on culturally appropriate care and community connection.

The Role
MiCare delivers a range of aged services for elderly people of Culturally and Linguistically Diverse (CALD) Background. Social Support Group is funded under the Commonwealth Home Support Program (CHSP), with the aim to assist frail, older people to participate in community life and feel socially included through structured, group-based activities that develop, maintain or support independent living and social interaction.
Key Result Areas
Applying person-centred, wellness and reablement approach to plan, organise and deliver activities to the group in order to provide social interaction for the participants.
Undertake comprehensive assessment of clients in collaboration with clients and carers. Develop individual profiles and care plans based on assessed needs and personal goals.
Support clients in developing and reviewing their goals in their care plan. Empowering them to live in community independently.
Support the Team by providing safe and reliable service for clients in accordance with program guidelines and procedures.

Demonstrated commitment to ongoing professional and staff development.
