The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

An opportunity has become available for an experienced eClinician to drive strategic projects and service improvement initiatives across our diverse and dynamic service portfolio. In this pivotal role, you will be primarily responsible for leading projects relating to clinical systems information management while also providing leadership and support to quality improvement initiatives across Primary Care and Community Health. You will play a key role in strengthening information management practices, enhancing data quality, improving performance reporting, and driving broader service improvement outcomes that support high-quality patient care.

Reporting within a collaborative and multidisciplinary environment, you will provide both strategic and operational leadership to deliver a portfolio of key projects and programs aligned with the objectives and timeframes of the PCCH Business Plan. Working closely with clinicians, managers, staff and consumers, the successful incumbent will support service redesign, improve service delivery, and promote a culture of service excellence, safety, continuous quality improvement and best practice.

The successful candidate will have demonstrated experience in project management, health information systems, service improvement and change management. You will possess strong analytical and problem-solving skills, with the ability to translate complex data into meaningful insights that inform decision-making and drive organisational performance. Your ability to build productive relationships and engage stakeholders at all levels will be critical to your success.
