The PCHN Safety, Quality and Performance team is evolving, with a strong focus on supporting frontline care delivery, strengthening contemporary governance and service improvement, and embedding robust safety and quality systems across Perth Children’s Hospital and Neonatology (PCHN).

About the Role

We are seeking an experienced Senior Clinical Incident Management Coordinator to provide strategic leadership across PCHN’s clinical incident and risk management program.

Reporting to the Director, Safety, Quality and Performance PCHN, you will work with senior clinical and executive leaders, governance committees and key stakeholders across clinical services and directorates. You will support timely, robust response to serious and high-risk incidents, facilitate complex incident reviews, and provide expert advice on open disclosure, clinical incident management, and investigation methodologies, while maintaining an independent, objective and evidence-based approach.  

A key focus of this role will be fostering a transparent and psychologically safe culture where staff feel supported to speak up, report incidents and contribute to organisational learning and continuous quality improvement. 

You will help identify, analyse and mitigate clinical and system wide risks, and translate incident findings and data into meaningful safety and quality improvements for patients, families and staff. 

Key responsibilities include 

• Leading and coordinating the PCHN Clinical Incident Management Program.
• Coordinate and facilitate incident investigations and complex reviews using contemporary investigation methodologies. 
• Analysing clinical incident data and complex information to provide clear insights into trends, emerging risks and opportunities for system improvement.
• Partnering with the directorates to develop and implement strategies that strengthen patient safety and clinical risk management across PCHN.
• Promoting organisational learning translating incident findings into practical and sustainable improvement.
• Developing and delivering education and capability-building in clinical incident management, investigation, open disclosure and clinical risk management.
• Driving a culture of learning, transparency and continuous improvement across PCHN, with a focus on communicating to speak up for safety, and report incidents and near-misses.  
• Contributing to the review and development of patient safety policies, procedures and governance frameworks.

If you have experience leading the formulation, development, implementation and evaluation of complex policies, frameworks or governance processes, together with a strong understanding of policy governance, compliance and the practical application of standards and legislative requirements, we encourage you to apply.

Our team works in partnership with the PCHN directorates (Medical, Surgical, Allied Health & Neonatology), clinical teams and CAHS colleagues to support safe, high-quality care. We lead and coordinate work across clinical governance, accreditation readiness, quality improvement, clinical incident and risk management, policy, safety and quality performance reporting and system-wide learning.
