ndis support coordinator

Support NDIS participants to understand and effectively utilise their NDIS plans.

Connect participants with suitable NDIS, community and mainstream services.

Coordinate supports and liaise with families, providers, allied health professionals and other stakeholders.

Promote participant choice, control, independence and capacity building.

Monitor plan implementation, funding utilisation and progress towards participant goals.

Identify service gaps, risks and changes in support needs and coordinate appropriate responses.

Assist with NDIS plan reassessments/reviews, reports and supporting documentation.

Maintain accurate case notes, participant records and required documentation.

Participate in stakeholder meetings and case conferences as required.

Work in accordance with the NDIS Code of Conduct, Practice Standards and Align Care policies and procedures.
