Support Coordinator - NDIS

We are seeking an experienced and motivated Support Coordinator to join our team.

This Position requires an Individual who has:

• Experience providing NDIS Support Coordination/Recovery Coaching Services or Similar Role 

• Support participants to understand and implement their NDIS plans

• Assist participants in building capacity, independence, and decision-making skills

• Coordinate and connect participants with appropriate service providers

• Monitor supports and ensure services align with participant goals and funding

• A Solid understanding of the NDIS and its Importance to People with a Disability and their Support Networks

• Demonstrated experience with Facilitating Multi-Disciplinary Coordinated Planning and Develop Collaborative Relationships and Networks between Service Providers

• Maintaining an Up-To-Date knowledge of the NDIS Systems and Practices

• Engaging with NDIA Staff and LAC’s

• Liaising with the Participant, Family or Representative on Supporting the Participant with Achieving their Goals

• Preparing for Plan Reviews and Attending with the Participant, Family or Representative

• Maintain accurate and compliant documentation in line with NDIS requirements
