About Us
Since its establishment in 1982, Alzheimer’s WA has served as a prominent advocate and source of support for individuals living with dementia, as well as their families and carers. We champion person-centred care, advocacy, education and comprehensive support through our Commonwealth Home Support Programme (CHSP), Support at Home (SAH) and day and overnight respite services across Perth, Mandurah and Albany.

About the Role
We are seeking an experienced and compassionate Care Coordinator to join our team in Mandurah on a 6-month fixed-term contract, providing coverage for a period of leave.
This is a varied care coordination role supporting clients across our Support at Home (SAH), Commonwealth Home Support Programme (CHSP) and other community-based services.
Working closely with clients, families, Support Workers, scheduling, clinical staff and external service providers, you will coordinate supports that are responsive to each client's individual needs, goals and circumstances.
This is a great opportunity for someone who enjoys working with people, coordinating services and making a positive difference in the community.
