This is more than a job. It's an opportunity to lead programs that help Aboriginal Elders, people living with disability, and their families navigate aged care, NDIS, health, and community services while strengthening outcomes for the Yalata community. The Community Connector Team Leader provides leadership to the Community Connector Team and oversees the delivery of the Elder Care Support and NDIS Remote Community Connector programs
Tullawon Health Service is an Aboriginal Community Controlled Health Organisation delivering culturally safe care and support for the Yalata community. Grounded in community, culture, family, and Country, we work to improve health and wellbeing outcomes while empowering people to remain connected to what matters most.
As the Community Connector Team Leader, you will lead a dedicated team delivering culturally safe navigation, advocacy, and coordination services across aged care, NDIS, health, and community support systems.
You will help ensure community members can access the services they need while maintaining choice, independence, and self-determination. You'll also build strong partnerships with service providers and advocate for improved outcomes for the community.
You are an experienced people leader who is passionate about community empowerment and improving access to services for Aboriginal people.
