As our services continue to grow across Tasmania, we are seeking an experienced and motivated NDIS Support Coordinator and Psychosocial Recovery Coach to join our dedicated team in Hobart.
About the Role
As an NDIS Support Coordinator and Psychosocial Recovery Coach, you will work closely with participants to help them understand and implement their NDIS plans, build their capacity, connect with appropriate supports, and achieve meaningful outcomes.
You will collaborate with participants, families, carers, allied health professionals, community organisations, and service providers to ensure participants receive coordinated, high-quality support tailored to their individual needs.
