360 Health + Community is an established, experienced and growing not-for-profit organisation delivering specialised Mental Health services, Headspace (x3), Suicide Prevention, Psychosocial Support, Chronic Health & Aged Care, Allied Health, and Street Doctor. We operate throughout the Perth metropolitan area, the Mid-West (out of Geraldton), and the Goldfields (out of Kalgoorlie).

About the role

The Peel Youth Mental Health Navigation and Coordination Service (PYMHNCS) is a new, innovative program and the Navigator will play a significant role in implementing and then providing outreach, support and connection services to the youth clients of this program. The Navigator will work within a multi-disciplinary team of psycho-social service navigators supporting youth who are experiencing mental health conditions and biopsychosocial complexities. This program will work in concert with the Wandjoo Gateway, and a significant part of this role is ensuring collaboration and integration with the network to achieve improvements in the mental health care of youth.

Key aspects of this role:

• Conduct initial assessments of needs and develop a Recovery Action Plan and associated risk and safety assessments.
• Provide one-to-one outreach support for youth with a diagnosed mental illness (or awaiting a diagnosis) enabling access and engagement with services identified in their Recovery Plan as being necessary to address their bio-psychosocial needs.
• Assist and guide youth with building a capacity to self-advocate or assertively communicate their needs.
• Communicate effectively and build positive relationships with clients, other health professionals and services, family members and carers.
• Facilitation of referrals and engagement with external organisations and services in support of the client’s needs.
  
  

·        What we are looking for / being successful in the role, you will have:

• Qualification in the mental health, community disability, or welfare sector (Certificate IV minimum, Diploma, Degree).
• Minimum of 1 years’ experience in direct service provision to young people and their families through a variety of programs and initiatives. This would include a strong understanding of how to work appropriately and effectively with young people aged 12 to 25 years.
• Demonstrated understanding of the issues and factors that influence the mental, physical, social, and emotional well-being of young people aged 12 to 25 years of age and their families. This would also include an awareness of the range of professionals that work within, or meet, the youth health sector.
• Empowering young people to participate in their own recovery and the ability to influence others to do the same.
• Understand recovery oriented, trauma informed, and client centred practices.
• Knowledge of mainstream health and community support organisations.
• Demonstrated ability in developing and interpreting care/action plans to support clients in achieving agreed goals.
• Current WA Driver’s License and access to a roadworthy, comprehensively insured vehicle.
• National Police Certificate - essential or willing to obtain
• Working with Children Check

An attractive remuneration package based on:

• starting from $39.94 per hour + superannuation +salary packaging.

Starting rate based on the qualifications and experience you bring to the role. Appointments are usually made at the starting rate for the band.

The role on offer is full-time and fixed-term to 30 June 2027. Positions are offered as full-time but applications for 0.8 FTE (4 days per week) may be considered for the right applicant.
