Aged Care Onsite Pharmacist - Full time or part time

As an Aged Care Onsite Pharmacist, you will work directly within our residential aged care partner facilities, becoming an extension of each customer's healthcare team.

Working alongside doctors, nurses, allied health professionals, residents and families, you'll provide expert clinical advice that supports the safe and quality use of medicines while improving resident outcomes.

No two days are the same. Your role will include a combination of clinical services, education, governance and quality improvement activities.

• Deliver onsite clinical pharmacy services within residential aged care facilities.

• Conduct medication reviews and support medication optimisation.

• Collaborate with GPs, nursing staff and allied health professionals to improve resident care.

• Promote the safe and quality use of medicines through evidence-based practice.

• Review medication management systems, policies and administration processes.

• Identify and reduce medication-related risks.

• Provide education and support to aged care staff.

• Support residents and families through medication education and communication.

• Contribute to clinical governance, quality improvement and compliance with Aged Care Quality Standards.

• Assist with medication supply processes to ensure residents receive timely access to medicines.

You are passionate about improving health outcomes and enjoy building strong professional relationships.

You'll also have:

• Current registration as a Pharmacist with AHPRA

• Experience in community, hospital or aged care pharmacy

• A genuine interest in clinical pharmacy and aged care

• A collaborative approach to working within multidisciplinary teams

• Excellent communication and relationship-building skills

• Strong clinical judgement and attention to detail

• A commitment to continuous learning and professional development

• Completion of (or willingness to complete) the Aged Care Onsite Pharmacist training requirements
