Clinical Care Partner (Aged Care) / Registered Nurse

We are seeking an enthusiastic and motivated Clinical Care Partner (Aged Care) / Registered Nurse to join our team.

This is a full-time position based in Warrnambool, with travel required across Western and South West Victoria. A current driver's licence is essential.

About the opportunity

As part of a collaborative and supportive team, you'll play a key role in partnering with other Care Partners and other employees with the purpose to deliver high-quality care and safe services.

Clinical Care Partners perform an integral role in ensuring individualised services are developed and delivered to participants based on their assessed clinical care needs, in consultation with participants, their family or guardians and key health professionals. Clinical Care Partners are responsible for conducting comprehensive nursing assessments and developing, monitoring & reviewing individual care plans and associated health care plans.

The Registered Nurse’s role is the efficient and effective delivery of professional nursing care and support to customers, respecting customer rights, and in consultation and cooperation with the customer, their representatives, their medical practitioner(s), and other health professionals.
