YW7 Community Health Nurse (Sole)

East Wimmera Health Service (EWHS) is a fully accredited small rural health service across five campuses encompassing the five communities of Birchip, Charlton, Donald, St Arnaud and Wycheproof, in North -western Victoria.

East Wimmera Health Service is now seeking a permanent full-time Central Intake Nurse to join our expanding Community Health team. Part-time hours will be considered.

As a member of a multi-disciplinary service, the Central Intake Nurse takes responsibility for referrals entering our Community Health services and ensures comprehensive health screening via telephone to determine; services and assistance required, priority, eligibility and costs of service.

The Central Intake Nurse will conduct comprehensive initial needs assessment (INI) of the restorative, physical, medical, psychological, cultural and social dimensions of care and provide consumers and carers with a choice of appropriate services, provide information and referrals and provide education and support to clients and their families on different pathways to receiving subsidised service.
