We are currently seeking a compassionate, organised and community-minded Palliative Care Volunteer Coordinator to join our team. This is a part-time position (28 hours per week), providing key support to the delivery of the Blue Mountains Palliative Support Service (BMPSS).

About the Role

Working as part of the Aged, Chronic Care and Allied Health Stream, you will contribute to participant and carer support coordination, volunteer recruitment and support, and the delivery of compassionate, person-centred services for individuals living with a life-limiting illness, along with their carers and families.

This is a hands-on role combining service coordination, volunteer engagement, community partnership development and program support, with a strong focus on responsive service delivery, continuous improvement and supporting positive outcomes for participants, carers and volunteers.

The position is based across Belong Blue Mountains neighbourhood centres in Blaxland, Lawson and Katoomba, with some community and home-based visits required.

Key Responsibilities

• Recruit, induct, train and support volunteers
• Coordinate and support the delivery of person-centred palliative care support services for participants, and respite support for their families and carers
• Manage participant referrals, intake processes and ongoing support coordination activities
• Develop and maintain strong working relationships with health professionals, Local Health District staff, community organisations and referral partners
• Maintain accurate records and meet reporting requirements
• Participate in community engagement, networking and partnership activities.

 About You

• You are a compassionate and community-minded professional passionate about supporting people and families navigating palliative care.
• You bring experience in community services, palliative care, aged care, care coordination or volunteer coordination, with strong stakeholder engagement skills.
• You are recognised for your ability to communicate with empathy, professionalism and sensitivity.
• You maintain strong organisational skills, accurate records and effective coordination across multiple priorities and locations.
• You are proactive, reliable and adaptable, with a commitment to quality service delivery and positive outcomes for participants and volunteers.

Essential Criteria

To be successful in this role, you will bring:

• Demonstrated experience in community services, palliative care, aged care, care coordination or volunteer coordination
• Prior experience supporting vulnerable participants, carers and families, preferably within the community sector
• Experience providing individual participant support across a variety of community and home-based settings
• Experience working collaboratively with external organisations and and/or health professionals
• Strong relationship-building and communication skills
• Strong organisational and administrative skills, including reporting and records management
• Ability to work independently and as part of a multidisciplinary team
• Relevant qualification in Community Services, Social Work, Allied Health, Nursing or related field, or equivalent experience
• Current NSW Driver Licence and access to a reliable vehicle
• Current National Police Check (or willingness to obtain)
• Current Working With Children Check (or willingness to obtain)
• Current First Aid Certificate (or willingness to obtain)
