Discharge Planner

Westmead Rehabilitation Hospital is seeking dedicated and enthusiastic Discharge Planner to join our dynamic team. Our well-established facility includes 65 rehabilitation beds, a hydrotherapy pool and fully equipped gyms, providing comprehensive care and support for patients throughout their rehabilitation journey.

Working closely with Case Managers and our multidisciplinary teams, you will coordinate safe, seamless and effective care transitions. You will work with patients, families, carers and external healthcare providers to develop personalised discharge plans that support each patient's rehabilitation goals and ongoing care needs.

Key Responsibilities

• Assess patient needs and discharge readiness from admission through to discharge.

• Develop personalised discharge and continuing care plans tailored to each patient's individual circumstances.

• Coordinate community services, support agencies and healthcare providers to facilitate successful transitions of care.

• Liaise with patients, families, carers, rehabilitation providers, aged care services, and other external stakeholders.

• Advocate for patient-centred outcomes by ensuring physical, emotional, cultural and healthcare needs are considered throughout the discharge planning process.

• Provide education, guidance, and resources to patients, carers and clinical staff regarding discharge pathways and community support services.

• Collaborate closely with medical, nursing, allied health and case managers to support continuity of care.

• Facilitate complex discharge plans while ensuring timely and safe patient outcomes.

• Maintain accurate and comprehensive documentation in accordance with hospital policies and professional standards.

About You

To be successful in this role, you will be a healthcare professional with experience in discharge planning, care coordination or case management within an acute, rehabilitation or healthcare setting.

Essential Criteria

• Current AHPRA registration or equivalent

• Relevant clinical knowledge, skills and experience developed within an acute care, rehabilitation or hospital environment.

• Experience in discharge planning, case management or care coordination.

• Strong understanding of aged care admission requirements and pathways.

• Knowledge of community services, patient support programs and available healthcare resources.

• Demonstrated experience managing complex discharges and coordinating multiple stakeholders.

• Excellent communication and interpersonal skills with the ability to build strong relationships with patients, families, carers, clinicians and external service providers.

• Well-developed critical thinking, problem-solving, and organisational skills.

• Ability to work proactively and independently while contributing effectively within a multidisciplinary team.

• Willingness to educate and provide advice to patients, families, and nursing staff.

• A compassionate, empathetic and patient-focused approach to care.
