Care Coordinator - Tuggeranong

About the role

The Care Coordinator will work as part of the GPH Aboriginal Health Team to improve health outcomes for Aboriginal and Torres Strait Islander communities across the ACT. The primary responsibility of the position is to provide ongoing care coordination and management of chronic and complex disease among Aboriginal and Torres Strait Islander communities. The Care Coordinator will work using a partnership and flexible approach with clients, Aboriginal Elders and communities, Aboriginal Community Controlled Organisations, General Practice, ACT Health and Allied Health providers.

This position is based in our Tuggeranong hub and is a full-time opportunity, fixed term until 30 June 2027.

Skills & Experience

• Current AHPRA registration as a Registered Nurse or Aboriginal Health Practitioner, OR Certificate III or IV in Aboriginal Primary Health Care with a minimum 2 years’ experience working in an Aboriginal Health Worker role.
• Experience working with Aboriginal and Torres Strait Islander communities and demonstrated cultural competence.
• Ability to work independently and confidence in exercising clinical judgement.
• Experience of the use of electronic clinical records (such as Best Practice), and knowledge of Microsoft Office including Word, Excel, PowerPoint, and Outlook.
