Job Description

• Part-time (0.6 FTE – 3 days) | 12-month contract
• Based at Ferntree Gully site, on the lands of the Wurundjeri People
• $40.49 - $44.29 per hour (subject to experience) + 12% Super + Salary Packaging benefits of up to $18,550 p.a.

Join Each's Linkages team and play a vital role supporting people with complex care needs to access practical supports, improve wellbeing and strengthen their connection to their community.

The Linkages Program provides short-term, individualised case management and flexible support services for people under 65 years of age (or under 50 for Aboriginal and Torres Strait Islander peoples) who are not eligible for the NDIS. Working across Melbourne's Outer Eastern suburbs, the team helps people overcome barriers, coordinate supports and access the services that matter most to them.

Your role, your impact
As a Linkages Case Manager, you'll work alongside people with complex support needs to create practical pathways that improve independence, health and wellbeing. Through assessment, care planning, advocacy and service coordination, you'll help people navigate systems, access services and achieve goals that are important to them.

You will:

• Conduct comprehensive assessments to understand individual goals, strengths and support needs
• Develop person-centred care plans that promote wellbeing, independence and choice
• Coordinate services and supports to ensure timely access to appropriate care
• Provide short-term case management responsive to the needs identified through assessment and care planning
• Build confidence and capability by providing health and social support education
• Advocate for customers and reduce barriers to accessing services and community supports
• Partner with families, carers and service providers to deliver integrated support
• Manage brokerage funds and service arrangements to meet customer needs
• Support transitions to NDIS, My Aged Care and other services where appropriate
• Maintain accurate documentation and reporting including onboarding, discharge and administrative processes
• Contribute to continuous improvement initiatives that enhance service quality and customer outcomes

About you
You'll be someone who enjoys building relationships, navigating complex systems and empowering people to achieve meaningful outcomes.

You have:

• A minimum of two years’ case management experience
• Working knowledge of services for younger people with disability and complex care needs
• Working knowledge of My Aged Care and services for older people
• Strong psychosocial assessment and person-centred care planning skills
• The ability to communicate respectfully with people from diverse communities and tailor your approach to individual needs
• Strong organisation and time-management skills, with the ability to work autonomously

Qualifications

Applicants must satisfy one of the following qualification pathways:

• Bachelor of Social Work OR
• Diploma or Advanced Diploma in Community Services, Case Management, Community Development, Disability, Ageing Support or a related human services discipline OR
• Demonstrated equivalent qualification and/or experience in case management within health, aged care, disability, or community services

Applicants must also have:

• Current driver's licence
• NDIS Worker Screening Check, or willingness to obtain
• Relevant professional registration (where applicable)

What will make you stand out

• Experience in case management, care coordination or community services
• Experience with CHSP, HACC, Home Care Packages and/or NDIS funding streams
• Previous experience managing brokerage budgets and service funding
• Knowledge of wellbeing and reablement approaches
• Understanding of aged care and community services legislation, standards and accreditation requirements
• Experience supporting people with low health literacy and those from diverse backgrounds
• Knowledge of Carelink Plus
